What is Methylene Blue?
Methylene blue, also known as methylthioninium chloride, is a synthetic compound with a long history of medical and scientific use.
In the UK, intravenous methylthioninium chloride has an established licensed medical use in the treatment of certain forms of methaemoglobinaemia, a condition affecting the blood's ability to carry oxygen normally.
However, methylene blue has also become the subject of wider scientific research because of the way it interacts with cellular metabolism and mitochondrial processes.
It is important to distinguish these areas of research from its established licensed medical indications. Research into a compound does not necessarily mean that a clinical benefit has been proven for every proposed use.
What are mitochondria?
Mitochondria are specialised structures found inside most of our cells.
They are sometimes described as the body's cellular “powerhouses” because one of their key roles is converting nutrients into adenosine triphosphate (ATP), which cells use as a source of energy.

Intravenous administration delivers the compound directly into the circulation.
Before treatment, an appropriate clinical assessment should be undertaken to establish whether the treatment is suitable for the individual.
This is particularly important with methylene blue because it can interact with certain medications and is not appropriate for everybody.
Why are people interested in Methylene Blue?
Much of the current interest surrounding methylene blue relates to research into three broad areas:
Mitochondrial function
Researchers have investigated how methylene blue interacts with mitochondrial respiration and cellular energy pathways.
Oxidative processes
Methylene blue's redox properties have led researchers to investigate its relationship with reactive oxygen species and oxidative stress.
Brain and cognitive function

One small randomised study involving healthy adults, for example, investigated a single low oral dose of methylene blue and reported changes in functional MRI responses during attention and memory tasks, alongside an effect on memory retrieval.
Importantly, this study involved oral methylene blue rather than a wellness IV treatment. It therefore should not be interpreted as proof that Methylene Blue IV Therapy improves memory, concentration or cognitive performance.
What happens during a Methylene Blue IV appointment?

If treatment is considered appropriate, methylene blue is administered intravenously according to the clinic's clinical protocol.
Because treatment protocols and individual circumstances vary, the dose and duration of treatment should be determined following clinical assessment rather than using a one-size-fits-all approach.
Is Methylene Blue suitable for everyone?
No.
Methylene blue can cause side effects and has important interactions with certain medications.
One particularly important consideration is its interaction with serotonergic medicines, including some commonly prescribed antidepressants.
UK medicines safety guidance has highlighted the potential for central nervous system toxicity when intravenous methylene blue is used in patients receiving serotonergic medicines.
Other factors, including certain medical conditions, pregnancy and G6PD deficiency, can also affect whether methylene blue is appropriate.
For this reason, anyone considering Methylene Blue IV Therapy should undergo an appropriate clinical assessment and provide an accurate list of their current medications and supplements.
Is Methylene Blue IV Therapy the same as taking Methylene Blue supplements?
No.
Intravenous administration and oral administration are different routes of administration and should not be considered interchangeable.
Research involving oral methylene blue cannot automatically be used to predict the effects of an intravenous treatment, particularly where doses, concentrations and patient populations differ.
